• Download a FREE PDF copy of the assessment tool.
  Allows you to download a copy of the assessment tool to share with key stakeholders in the practice (limited to a single use in a single practice site). After reviewing the questions you can come back to the site to complete the online assessment, if you wish (see below).
  
  
• Purchase and complete the assessment ONLINE and access the comparative report. Cost: $200 per practice site*
  Completing the Physician Practice Patient Safety Assessment online is necessary in order to receive a copy of the benchmarking report. This is a key benefit that will allow you to compare your responses with others who have completed the assessment. After completing the assessment and entering your results online, you will be able to print a copy of the assessment with your scores for each item, domain and total score. You can then compare your scores with the aggregate results from other practices. If you have more than one practice site, a separate assessment should be completed for each.
